AHRC NYC is looking to hire a Complex Needs Care Coordinator for their partnership with the NYC Department of Social Services to help coordinate care for people with IDD living within the NYC Shelter system.
This is a grantfunded public/private partnership between AHRC New York City and the NYC Department of Social Services (DSS) which includes the NYC Department of Homeless Services (DHS) and NYC Human Resources Administration (HRA). The Program seeks to (1) develop a digital toolkit including a collection of evidencebased resources methods and strategies to help NYC shelter staff recognize and appropriately coordinate care for people with I/DD in the NYC shelter system; (2) curate a series of trainings related to a variety of topics such as how to recognize signs and behaviors of people with I/DD effective communication techniques sensory integration techniques for prevention of disruptive behavior and increasing physical accessibility and access and (3) work with NYC shelter systems and shelter provider agencies seeking help for a person who is in crisis and homeless with I/DD or suspicion of I/DD to assess needs develop a plan of care and follow up to ensure the person is connected with services.

ESSENTIAL RESPONSIBILITIES
- Work in collaboration with NYC DSS and NYC DHS team members to support people with I/DD or suspected I/DD in the NYC shelter system. Assess their identified emotional social vocational and housing needs during and in the aftermath of crisis/emergency episodes and develop a plan of care to connect with needed services and supports.
- Develop training series for DSS shelter staff in identifying and supporting people with I/DD or suspected I/DD on a variety of relevant topics. This training will also include information on how to obtain OPWDD eligibility and access OPWDD services.
- Develop a digital toolkit for designated DSS shelter staff with evidencebased resources methods and strategies to support people with I/DD.
- Provide technical assistance to shelter staff as needed and appropriate for people with I/DD or suspected I/DD.
- Coordinate evaluations needed for OPWDD eligibility; this can be done onsite in the shelters or through a clinic depending on the needs of the person.
- Provide referral and case coordination services to assist DSS staff in helping people with high needs/ complex living situations.
- Performs all duties necessary for compliance with relevant regulations related to service provision including record keeping.
- Track information needed for grant reporting.
- Work with the DSS Office of Evaluation and Research (OER) on developing a tool to evaluate the program.
- Provide regular written reports to appropriate stakeholders and constituents including required reports to the grant funder.
- Attend all required meetings.
- Conducts other related tasks as assigned.
Qualifications :
REQUIRED QUALIFICATIONS
- Bachelors degree (BA BS) Masters preferred in a Human Services discipline from an accredited university program required.
- Experience with and excellent knowledge of the OPWDD services system and strong working knowledge of eligibility process.
- Experience with crisis management and working with people with intellectual and developmental disabilities.
- Strong interpersonal written and verbal communication skills.
- Excellent skills in using MS Office applications as well as agencyspecific database systems.
- Proven capacity to work productively in highly collaborative settings and to coordinate the needs of multiple stakeholders.
- Ability to make decisions independently and to react to unanticipated events in prudent and timely manner.
- Flexibility to operate in a rapidly changing work environment with the ability to adapt and be sensitive to a broad spectrum of locales and circumstances.
- Must be willing to travel within five boroughs of New York City. Valid NYS Drivers License a plus.
