The OB practice area is made up of 2 OB/GYN 2 FPBO and 2 CNM. St. Elizabeth is a critical access hospital located in a beautiful community setting.
Rate Caps for Submission:
Malpractice Must be included in rates.
Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly under BR75 Per hour not OT
Holiday Must be 1.5x the regular rates Per holiday hour no OT
On Call (Daily) under BR Per 24 hours to include 4 hours gratis
24 Hour Holiday Call Rate BR500 Per 24 hours to include 4 hours gratis
Call Back under BR75 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Holiday Call Back BR Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Average worked Callback Hours: weekday 34h weekend 12h; weekday less than 50% weekday weekends and ER.
Schedule/Availability Requirements:
78 call shifts/month can be combined with clinic days
3 days of clinic per week 3 weeks per month preferred
Multiple part time locums accepted to fill schedule: Site prefers just one for clinic consistency willing to consider locums for just call or clinic.
Shift Details: 24 hr call 7a7a with 20 min response time. Contact by cell phone. Clinic 8a5p MondayFriday.

Setting: Critical Access Hospital and Clinic
Duties: OB/GYN clinic care OB and GYN call coverage through L&D Unit and ED.
Required Procedures: CSection Ectopic
EMR: Epic
Support Staff: MA or LPN in clinic. L&D RNs at hospital
Reason for Coverage: Medical Leave
Dealbreakers: Low risk high risk and FHR interpretation required. Routine delivery and complicated delivery. Repair of 3rd4th degree lacerations. Csections must be willing to help and supervise if needed. Must be comfortable handling OB inductions from group mostly emergent GYN surgeries.

Travel Preferences (see attachment for specific travel guidelines):
Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market.
Air Travel: BR50/round trip cap
Rental Cars: BR/day all in cap (including taxes and fees) regardless of car class.
Mileage: IRS Standard Rate
Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Credentialing Information:
Timeframe: 3 months
Credentialing with other facilities locums automatically credentialed at all facilities not including St. Michaels.

Additional Details / Questions Answered by Facility:
25 bed critical access hospital with 3 L&D beds and 4 post partum beds. Level I Nursery Level I Trauma only 36 weeks and above.
LI Nursery: late term and term deliveries. Less than 50% callback weekday weekends and ER. 90% low risk pregnancy 10% high risk 20% surgical gyn. No other OB/GYN present during locum coverage. Low risk high risk and FHR interpretation required. Routine delivery and complicated delivery. Repair of 3rd4th degree lacerations. GYN ER normal scope of practice DCs Ectopics ER consults etc. High risk patients get transferred to Tacoma. Peds currently done by CNMs/FPOBS. LD staffed with 23 RNs. 3 operation rooms on 2nd floor. L&D 3rd floor. CSections recovered in LD. Providers include CNMs FPOBs who do CSections. Must be able to help and supervise if needed. Anesthesia CRNAs and OR staff have 30 min response time. Currently doing VBACs. Will occasionally receive transfers from local lay midwives. Must be comfortable handling OB inductions from group mostly emergent GYN surgeries. 2030 deliveries/month high risk 10 or less.
Typical Cases: OB: low risk/high risk ultrasounds amniocentesis. L&D: routine/complicated VBACs CSections. GYN: GYN Surgery
