We are seeking a dedicated Hospice RN Case Manager to provide comprehensive care coordination and case management services to hospice patients and their families. The ideal candidate will be responsible for assessing patient needs developing care plans and ensuring that patients receive appropriate services in alignment with their care goals. This position requires strong communication skills the ability to work independently and a passion for delivering compassionate endoflife care.
Key Responsibilities:
Care Coordination: Develop and manage individualized care plans for hospice patients ensuring that all aspects of care (medical psychological social) are addressed and aligned with the patient s needs and wishes.
Patient Assessments: Conduct regular assessments of patients medical emotional and social needs and update care plans as necessary.
Collaboration: Work closely with physicians nurses social workers chaplains and other members of the interdisciplinary team to ensure comprehensive care delivery.
Advocacy: Act as a patient advocate helping families navigate complex healthcare decisions and ensuring the patient s voice is heard in the care process.
Education and Support: Provide education to patients and families on hospice services symptom management and coping strategies. Offer emotional and psychological support to both patients and their families.
Documentation: Maintain accurate and timely case management records including assessments care plans and progress notes in compliance with regulatory guidelines.
Compliance: Ensure that all care services meet industry standards and regulatory requirements for hospice care.
Discharge Planning: Assist with transition plans for patients as necessary including discharge or transfer to another care facility when appropriate.
