Job Description
Job Title: Travel RN Care Manager (Home Health)
Job Description
Key Responsibilities
- Perform comprehensive assessments of patients in their home environment to develop individualized care plans.
- Collaborate with multidisciplinary teams to coordinate patient care and services.
- Monitor patient progress and adjust care plans as necessary to meet health goals.
- Educate patients and families about managing their health conditions and medications.
- Document patient interactions assessments and care plans accurately and timely.
- Travel to patients homes as required; mileage reimbursement at government rates.
Requirements
- Current Registered Nurse (RN) license in California.
- Minimum of 2 years of experience in Home Health preferred.
- Clean driving record is required.
- Basic Life Support (BLS) certification.
Additional Information
- Work Schedule: Day shift 40 hours per week.
- Contract Duration: 26 weeks.
