Job Description
Summary: Looking for a full time bread and butter Urologist for clinic and call. Clinic Manager will provide clinic tour/orientation before start date. Applicants should be interested in a wide variety of general cases clinic rounding and OR. Candidates should be collegial team players with good patient and customer service skills. Robot onsite at hospital for cases if needed. Will be sole provider in a fast paced/busy dept. Provider may take hospital call during clinic hours.
Assignment Duration: 34 months

Rate Caps for Submission:
- Malpractice Must be included in rates.
- Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
- Hourly under xx350 Per hour not OT
- Holiday Must be 1.5x the regular rates Per holiday hour no OT
- Nightly Call under xx700 Per night call to hold the pager to include 2.5 hours gratis
- Holiday Nightly Call xx1050 Per night call to hold the pager to include 2.5 hours gratis
- On Call (Daily) under xx2900 Per 24 hours to include 4 hours gratis
- 24 Hour Holiday Call Rate xx4350 Per 24 hours to include 4 hours gratis
- Call Back under xx350 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
- Holiday Call Back xx525 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

4/730 excluding weekends
Minimum length of commitment required: 3 months
Minimum number of shifts preferred/month: 56 days of call plus clinic
Specific/ugent dates needing coverage: asap
Est. days per month needing covered: clinic access as many days as possible 24 call 56 days per month
Remote work will include inbasket coverage and triaging patients on schedule to determine urgency/need.

Rounding: Locum will round

Reason for Coverage: open position with difficulty in recruiting current locum unable to keep up in fast paced setting.
Dealbreakers: Providers who are not agreeable to terms in the job desc. Providers who cannot keep up in a fast paced busy enviornment providers who are not agreeable to rates or who have not reviewed the job description and understand pt. volumes and setting.

Requirement description :
- Board Certification REQUIRED (Can accept Board Eligible)
- WA State License REQUIRED will accept providers with active IMLC
- Clean Malpractice PREFERRED
- BLS REQUIRED

- Specific dates and black out dates required for presentation REQUIRED
- Must list # of shifts provider is available to offer/month & if availability is shared between assignments REQUIRED
Vendors are now required to compile all presentation documents for a locum into a single PDF file.
The PDF must be uploaded into TRIO in the Candidate Documents section under the locums submission page.
The PDF title should include the locums name site location and specialty (ex. Dr. John Smith Good Samaritan Physician Nephrology)
An uptodate CV (solely) is still required to submit in Name Clear Documents initially when sending a new name clear. Vendors will have until bid due date to submit a PDF of ALL presentation documents. If PDF is not submitted TRIO will not clear provider.
Required Documents for a Full Presentation Packet:
Uptodate CV (Gaps greater than 30 days will need notation/explanation of gap in work history)
Malpractice documentation (NPDB and or write up). Must include ALL cases dates and payouts.
Disciplinary Actions (Must provide a detailed explanation)
