Clinical Leadership and Compliance: Supervise daytoday clinical and office operations. Be available to staff and patients during hours of operation. Ensure services are in compliance with professional standards agency policy and state and federal regulatory requirements. Serve as backup support for positions as needed: PCM CSS. Lead in the absence of the Territory Administrator.
Clinical Oversight of Patient Care: Coordinate referrals. Coordinate patient care with an interdisciplinary team approach. Assure the development implementation and updating of the individualized plan of care. Evaluate the quality of clinical care ensuring that patient needs are adequately assessed. Collaborate with clinicians and physicians regarding patient transfers or discharges. Review discharge assessment data Edit/Lock Discharge OASIS and disciplineonly discharge. Manage daily KPIs workflow escalation/complaint calls. Guide professional clinicians on implementing Emergency Care Protocols.
Clinical Oversight of Personnel: Assure clinicians including contract staff have required training competency and supervision. Make patient and personnel assignments ensuring that assigned staff have demonstrated competence in the required tasks/services. Provide ongoing coaching and mentoring of field clinicians.
Quality and Compliance: Act as QAPI owner and participate in QA and performance improvement activities. Review/investigate incidents analyze for trends and implement corrective action plans as needed. Ensure Accreditation/Survey Readiness perform QA audits/detailed corrective action plan audits. Manage HHCAHPS distribution in SHP. Participate in Emergency Preparedness Planning – Activation Reporting & Ongoing Event Monitoring and Reporting.
Experience you will need:
Bachelor’s degree in nursing healthcare administration or a related field; Masters degree preferred.
Current nursing license or relevant clinical licensure.
HH leadership experience and OASIS experience.
QAPI/Survey Readiness (is a plus).
Proven experience in a clinical managerial role or similar position.
Strong understanding of healthcare regulations compliance standards and quality assurance.
Excellent leadership organizational and communication skills.
Ability to analyze patient care data and implement evidencebased practices.
Proficient in electronic health record (EHR) systems and healthcare technology.
