Location: St. Martins Extra Care Queens Street Gillingham

  • 12.64 13.35 per hour
  • Part time contract 25hours per week

About the role:

The Community Connections Coordinator will enable the people we support to access local services activities and events and provide signposting and guidance as required. It is the coordinators responsibility to develop activities and deliver opportunities which reflect individuals wishes and their needs as identified through meetings and reviews. This included facilitating and supporting individual and group activities within St Martins as well as within the local community.

The role is focused on enabling people to live their lives as independently as possible and supporting them in reaching their potential through practical and emotional support.

Main responsibilities:

  • Support the Manager and care staff in the arranging and delivery of activities to a variety of people we support within a residential or day care setting.
  • Develop activities which reflect wishes of the people we support and their needs as assessed by care staff in order to meet aims and objectives as outlined in care plans.
  • Run and supervise individual and group activity programmes.
  • Maintain records for the people we support and contribute to other establishment records as required.
  • Provide personal care to the people we support as required.
  • Undertake the direction of volunteers.
  • Contribute to the development and review of individual care plans to meet needs for social and mental stimulation of the people we support.
  • In conjunction with the Manager organise and arrange group and individual activity programmes and to undertake risk assessments in relation to all planned activities.
  • Participate in staff meetings and undertake training.
  • Undertake any other duties of a lesser or comparable nature as required.

About you:

Previous experience as a Care Assistant or Activities Officer is beneficial as this provides knowledge of the care setting. However we will consider applications without this experience for the right person as all training will be provided.

Successful candidates must have:

  • An understanding of personcentred working and how to work with people to understand their priorities and interests
  • Experience of planning and organising community involvement
  • Good organisation skills
  • A solutionfocused attitude
  • An ability to adapt to the differing needs of individuals
  • A creative energetic and proactive approach
  • A desire for people to do the things that matter most to them.

Good qualities to have:

  • A compassionate and caring nature with a genuine desire to help others
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Creative with the ability to provide unique ideas
  • Organisational skills in order to arrange and plan multiple activities including group activities and larger events
  • Basic knowledge of health and safety
  • Awareness of antidiscriminatory practice/attitudes
  • Some experience of working in a caring or role in the community
