Role Overview:
Are you a compassionate healthcare professional with a strong ability to assess and manage nursing home care while providing oncall support as needed SereneAid is seeking a Nursing Home Care Manager (OnCall) to join our team. In this role you will be responsible for evaluating resident needs overseeing care delivery and leading the team of caregivers all on an oncall basis as determined by assessment and immediate care needs.
Responsibilities:
Resident Care and Support:
OnCall Assessment and Care Delivery:
- Provide oncall support for nursing care needs responding to assessments and requests as required. Evaluate resident conditions and determine the level of care needed in realtime.
- Conduct assessments when necessary (e.g. changes in resident health emotional needs or social factors) and ensure the delivery of appropriate care during your oncall shifts.
Care Plan Evaluation and Adjustment:
- Review and adjust care plans based on ongoing assessments. Ensure each residents care needs are met adjusting plans to reflect changes in health preferences and conditions.
Collaboration & Communication:
- Coordinate with healthcare professionals family members and residents during your oncall shifts to provide seamless holistic care based on assessments.
- Respond to urgent situations promptly making quick informed decisions to ensure resident wellbeing.
Team Leadership and Support:
- OnCall Staff Supervision:
- Offer leadership and guidance to caregiving staff when oncall making decisions about care delivery staffing and delegation of responsibilities.
- Monitor caregiver performance and conduct adhoc evaluations during oncall hours to ensure that care is provided according to SereneAid standards.
- Staff Development:
- Assess staff training needs during oncall shifts and provide feedback to improve team performance and care outcomes.
- Work with the management team to identify areas for staff improvement based on oncall performance evaluations.
Family and Community Engagement:
- OnCall Communication with Families:
- Build relationships with families through regular updates during your oncall shifts. Assess family concerns or changes in care needs and incorporate feedback into care planning.
- Community Outreach:
- Support community initiatives on an asneeded basis during oncall periods liaising with community organizations and healthcare providers to enhance resident care and support services.
Operational Excellence:
- Compliance and Standards Review:
- Ensure that care provided during oncall hours adheres to regulatory standards and SereneAid s policies.
- Conduct assessments of operational procedures to maintain compliance and identify areas for improvement during oncall shifts.
- Resource Management:
- Use assessments to determine resource needs (e.g. staffing supplies) during your oncall hours ensuring that operational efficiency is maintained even during shifts.
Health and Safety:
- Safety Protocols & Risk Management:
- Assess and address safety concerns during oncall shifts. Ensure health protocols are followed and that emergency preparedness plans are in place for any urgent incidents.
- Evaluate and mitigate risks to resident health and safety including infection prevention measures as part of your oncall responsibilities.
