Job Description
This facility will not consider any local traveler who resides within 50 miles of facility
This need is located at Jaffrey Physical Therapy Clinic.
- Provides patient/resident therapy evaluations and/or treatments according to established policies and procedures for pediatric to geriatric patients
- Outpatient setting with ortho neuro and general medical patient caseload.
- This position will cover shifts in outpatient Physical Therapy.
Patient ratios can vary but typically 1215 patients per shift.
Patient population: AdolescentGeriatric
Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!!

Nonbillable orientation: 8hrs
OT Holiday multipliers: 1.25%

Requirement description :
- Minimum 2 years CURRENT PT experience Required NO NEW GRADS
- Outpatient experience Required
- Inpatient experience Preferred
Profile Requirements:
- Cover page if applicable (not required)
- Work History
- Skills Checklist current to 1 year pertinent to specialty
- 1 Supervisory Signed Reference from the past two years of employment
- License/Certifications (NH license (or compact) BLS Facility will not pend license/certs)
CERTIFICATION REQUIREMENTS :
- BLS (AHA)
STATE LICENSE REQUIREMENTS :
- New Hampshire
ADDITIONAL LICENSE REQUIREMENTS : NH License preferred Compact state will also be considered
Weekend Requirements : Occasional weekend for Inpatient coverage/Holiday coverage required if needed
On Call Requirements : None
