Position Overview We are seeking a dedicated and experienced Care Management Manager to ensure the delivery of quality services in compliance with hospital standards and regulatory requirements. This position involves overseeing department operations managing personnel budgeting quality assurance and participating in performance improvement initiatives. The role also requires fostering positive team collaboration and maintaining professional relationships with external community agencies.
Key Responsibilities Operations Management - Ensure department services align with hospital mission and regulatory standards.
- Develop implement and monitor departmental goals objectives and quality improvement plans.
- Oversee budgets staffing and resource allocation to meet patient care and operational needs.
- Maintain compliance with state local and federal regulations.
Team Leadership & Development - Manage recruitment orientation training performance evaluation and continuing education for staff.
- Conduct regular team meetings address concerns and document minutes for future reference.
- Foster a culture of positive employee relations customer service and professional growth.
- Serve as a mentor and provide coaching to staff to promote professional excellence.
Performance Improvement - Participate in and lead departmental and hospitalwide performance improvement initiatives.
- Develop and implement departmentspecific quality assurance and PI plans.
- Monitor department performance metrics and identify areas for improvement.
Regulatory Compliance - Stay current on Medicare Medicaid and private insurance guidelines.
- Ensure compliance with accreditation standards and departmentspecific regulations.
- Utilize Milliman/InterQual criteria for case management processes.
Collaboration & Communication - Collaborate with interdisciplinary teams to support a cohesive care delivery model.
- Maintain strong relationships with community agencies to ensure seamless patient care transitions.
- Serve as a role model and advocate for hospital initiatives and quality standards.
Qualifications Education & Licensure:
- Bachelors Degree in Nursing with an active RN license (required).
- Masters Degree in Nursing (preferred).
- Certified Case Manager (C.C.M.) and PRI/Screen certification (preferred).
Experience:
- 5 years of managerial experience in an acute care hospital setting.
- Previous case management experience required.
- Knowledge of healthcare financials regulatory standards and payer systems.
- Experience using Milliman/InterQual criteria.
Skills:
- Proficiency in Microsoft Office and data analysis.

Primary Population Served: All patient populations including neonatal pediatric adult and geriatric.
