Join Our Team and Make a Difference in PostAcute Care
We are seeking a compassionate and experienced Internal Medicine Physician to join our dynamic PostAcute Division. This fulltime position offers a rewarding opportunity to provide highquality care to our skilled nursing facility patients in the St. Augustine and Jacksonville areas.
Internal Medicine Position Responsibilities:
- MondayFriday rounding on 12 buildings per day depending on census.
- NP support to assist with patient care.
- Manage a patient load of 2022 patients per day.
- Utilize Nursing Home EMR for efficient charting.
- Provide comprehensive care to skilled shortterm patients with 3 visits per week.
- Attend to 35 buildings as needed.
- Day call coverage from 8 AM to 6 PM for facilities rounded on that day.
