Job Description

Job Title: Licensed Social Worker

Job Category: Mental Health Professionals

Location: Tucson Arizona

Job Summary

We are seeking a compassionate and skilled Licensed Social Worker to join our team in Tucson Arizona. In this role you will provide comprehensive support to Veterans and their families by addressing their psychosocial mental health and crisis intervention needs. The ideal candidate will bring expertise in biopsychosocial assessments discharge planning and interdisciplinary collaboration while demonstrating a commitment to excellence in patient care.

Key Responsibilities
  • Assessments and Planning:

    • Conduct comprehensive biopsychosocial evaluations to identify patient needs.
    • Develop and implement care plans addressing social determinants of health.
    • Perform suicide risk mental health and crisis assessments and provide appropriate interventions.
  • Patient Support:

    • Document safety plans and interventions in medical records.
    • Educate and assist patients with completing advanced directives.
    • Coordinate and process inpatient psychiatric petitions when required.
  • Collaboration:

    • Communicate critical patient information with interdisciplinary teams.
    • Facilitate family and team meetings including discussions around goals of care.
  • Discharge Planning:

    • Create discharge plans including referrals to rehabilitation longterm care or palliative care facilities.
    • Plan for home health services and other resources for Veterans returning home.
    • Assist homeless Veterans with resources and coordinate with Adult Protective Services for abuse or neglect cases.
  • Case Management:

    • Manage postdischarge case assignments providing ongoing support and services.
    • Educate patients and families on care plans ensuring understanding and compliance.
  • Crisis Response:

    • Support emergency measures and provide lifesaving interventions when necessary.
    • Maintain professional decorum while respecting patient rights and privacy.
Required Skills and Experience
  • Minimum of 2 years of current social work experience preferably in mental health or crisis intervention settings.
  • Strong skills in:
    • Crisis intervention and mental health assessments.
    • Discharge planning and connecting patients to appropriate resources.
    • Interdisciplinary collaboration and patient advocacy.
  • Proficiency in working with electronic medical records and documentation protocols.
Educational and Certification Requirements
  • Education: Master of Science in Social Work (MSW) from a nationally accredited institution.
  • Licensure: A current full and unrestricted social work license in any U.S. State Territory or the District of Columbia.
  • Certification: Basic Life Support (BLS) certified by an American Heart Association (AHA)approved vendor.
