Job Description
We are seeking a dedicated and compassionate Occupational Therapist to join our skilled nursing facility (SNF) team in Lodi CA. This role offers the opportunity to work in a supportive environment focused on providing highquality patient care.
Responsibilities:
- Conduct patient evaluations to assess their physical and mental abilities
- Develop and implement individualized treatment plans to improve patients functional abilities
- Provide therapeutic exercises and activities to enhance patients physical and cognitive skills
- Educate patients and their families on home care instructions and adaptive equipment
- Collaborate with other healthcare professionals to ensure comprehensive patient care
- Maintain accurate and timely documentation of patient progress and treatment plans
- Participate in team meetings and contribute to the development of patient care strategies
Qualifications:
- Current state licensure or eligibility for licensure in California
- Strong clinical skills and knowledge of various therapeutic techniques
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Commitment to providing highquality patient care and continuous professional development
Schedule:
- Fulltime 40 hours per week
- Monday to Friday 8:00 AM to 4:30 PM
- No weekend rotation

- Skilled Nursing Facility (SNF) with 149 beds
