Job Description
We are seeking an experienced and compassionate Home Health Admission RN to join our team. The ideal candidate will have a strong understanding of home health nursing and a commitment to providing highquality care to patients in their homes.
Responsibilities:

Conduct comprehensive patient assessments to determine eligibility for home health services
-
Develop and implement individualized care plans for patients
-
Coordinate care with other healthcare professionals such as physicians social workers and physical therapists

Educate patients and their families about home health services and selfcare management
-
Monitor patients progress and make necessary adjustments to their care plans
-
Document all patient care activities in accordance with company policies and procedures
Qualifications:
-
RN license in the state of State
-
2 years of experience in home health nursing
-
Strong assessment and care planning skills
-
Excellent communication and interpersonal skills
-
Ability to work independently and as part of a team

Proficiency in electronic health records systems
Actual pay will be adjusted based on experience and other qualifications.
