Job Description

Position: RN Community Health

Start Date: 9/23/2024

Duration: 13 Weeks

Location: Pittsfield MA 01201

Shift Type/Shift Times/Hours/Day: Days 8:00 AM 4:30 PM 8

Shift Notes:8hr days 40hr guarantee

Weekend Requirements: Every 23 weekends/Holidays required as needed

OnCall Requirements: Possible call as needed

COVID Vaccination: COVID VACCINATION (dose date on or after 10/3/23) NOW REQUIRED FOR THIS HEALTH SYSTEM. Exemption forms are available in Downloadable Forms (Both forms REQUIRE Provider/Religious Leader Signatures) or please ask your delivery team for documents. IF APPROVED BY THE HEALTH SYSTEM TRAVELER WILL WEAR A MASK AT ALL TIMES WHILE ON SITE. To avoid delays in processing please include the travelers COVID vaccination card/or intent to fill out the exemption form with profile submission.

Travel Policy: Candidates Permanent Address (Facility will not consider local candidates 50mile radius restriction)

The Community Health Nurse (CHN) provides teaches supervises and documents nursing care for the sick and injured at home.

  • The CHN intervenes to promote maintain or restore health to prevent illness and to effect rehabilitation
  • The CHN collaborates with other healthcare providers professionals and community representatives to coordinate care at home
  • The CHN evaluates the individuals and families response to interventions.
  • Ability to function independently under the general guidance of the Director and follow the guidelines and protocols and/or policies and protocols specific to each unit
  • Duties require independent action and judgment
  • Utilizes time management communication planning delegating and problemsolving in providing care.
  • Must possess and demonstrate critical thinking skills. Must maintain emotional stability in emergency settings.

Also required:

  • Ability to exercise independent judgment.
  • Demonstrated interpersonal skills.
  • Ability to supervise others
  • Ability to maintain confidential information
  • Ability to develop and maintain records and communicate clinical data in verbal and written form
  • Willing to be flexible regarding work hours.
  • Ability to drive an automobile in inclement weather.
  • Must be willing to accept oncall responsibilities.
  • Conforms with Customer Service and Performance Standards.
  • Conforms with Patients Rights Policy.
  • History of dependability in meeting demands of committed work schedule.
Submission Requirements
  • One year of acute nursing care is required hospitalbased preferred.
  • Experience in home health Required
  • Certified Home Health Required Including Proficiency with OASIS
  • Infusion Therapy Certification Preferred
  • Demonstrated knowledge of the principles and practices of nursing.
  • Knowledge of and ability to apply community health principles and practice.
  • Demonstrated ability to perform independently as a clinician with minimal supervision or assistance.
  • Experience with selfscheduling of caseload preferred.
  • History of flexibility in assuming varied assignments.
  • BSN strongly preferred
  • Current Drivers License (and insurance) Required
  • To effectively provide home care they must have a working knowledge of home care reimbursement and regulatory guidelines.
  • Traveler candidate must have their vehicle. This is a firm requirement.
