Job Description

Position Name: Registered Nurse Hospice / Home Health / Case Manager Immediate Opportunity!

Location: Littleton NH 03561

Start Date: ASAP

Employment Type: Fulltime Travel Contract Temporary

Contract Length: 13 weeks contract Accepting Local and Travel Candidates

Pay Rate: $2200 to $2480 Weekly Gross pay (Nontaxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered from office to patient home visits and in between patient home appointments

Unit: Home Health Hospice Case Management

Shift: 8 Hours; Days 8.00 am 5.00 pm

Schedule: 40 hours / week guaranteed 5 shifts per week MonFri No weekends No holidays No call

Position Requirements:

  • Registered Nurse: 2 years Required
  • Current experience Hospice Home Health and Case Management services: 2 years Required
  • Must have experience providing care for patients of all ages
  • NetSmart.All Scripts EMR experience Highly preferred
  • Must have valid Drivers License and Personal Auto Insurance

Education License and Certification Requirements:

  • ASN or an equivalent Required
  • BSN or an equivalent Highly preferred
  • RN License for State of NH or Compact Required
  • BLS Certification from AHA or American Red Cross Required

Job Description:

  • Provide home health hospice nursing and case management services for patients in comfort of their homes.
  • Develop and implement nursing care plans including Start of Care and admissions as assigned.
  • Support patient comfort and ability to stay at home enabling them to spend quality time with their loved ones and doing the activities they enjoy.
  • Empower patients and families to reach healthcare goals by educating them on disease management.
  • Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations policies procedures and protocols as well as facility and corporate policies and procedures.
  • Ensure physicians orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting for all patient visits in timely manner.
  • Provide bereavement support to patients families as needed.
  • Area of Coverage: Whitefield/Berlin NH area
