Job Description

Currently we are partnered with a premier Arizonabased acute care hospital in search of a visionary Director of Case Management to lead their comprehensive care coordination program.
Job Summary:
Lead and transform our clients case management department while driving exceptional patient outcomes through strategic oversight of utilization review discharge planning and care coordination. Key Duties & Responsibilities:
- Drive strategic direction of case management services across the facility
- Lead and mentor a multidisciplinary team of case management professionals
- Optimize care coordination processes to enhance patient outcomes and satisfaction
- Develop and implement innovative care management protocols
- Ensure compliance with regulatory requirements and accreditation standards
- Partner with executive leadership to achieve organizational objectives
- Manage department budget and resource allocation effectively
- Lead quality improvement initiatives and outcome measurements
Qualifications:
- Bachelor of Science in Nursing required (Masters degree highly valued)
- Active Arizona RN license or Compact State license
- Current BLS certification
- Proven success in case management leadership
- Strong understanding of healthcare regulations and reimbursement models
- Experience with Interqual or MCG criteria software
- Advanced proficiency in Microsoft Office suite
- CCM or ACM certification valued
