Job Description



Summary: Holy Cross Hospital Jordan Valley West (HCW) is looking for an OBGYN to help cover 1 week per month of clinic coverage MonFriday 9a5p.


BID DUE DATE: Oct 30th @ 5p EST!


Rate Caps for Submission:

Malpractice Must be included in rates.

Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly BR350 Per hour not OT

Holiday Must be 1.5x the regular rates Per holiday hour no OT


Schedule/Availability Requirements:

  • Start Date: January 2025
  • Assignment: OB/GYN physician covering 1 week per month of clinic coverage MonFri only 9a5p.

Details:

  • Clinic (MonFriday 9a5p)
  • Facility Location: 3336 S 4155 W Ste 301 West Valley UT 84120
  • Setting: Outpatient Clinic Trauma Level III
  • FTE: 2.0
  • Required Procedures/Job Duties: General OB/GYN procedures & job duties. Candidates will need to see patients in clinic.
  • EMR: Epic
    Support Staff: MA and Sonographer in the clinic
  • Reason for Coverage: Departure of existing physician and another physician ramping down in preparation for retirement.


EMR Required Training:

  • Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
  • EMR Virtual training Only able to attend if Modules are completed before training. Provider is required to schedule and complete their virtual EMR training prior to their first day to avoid missing their first shift.

Travel Preferences (see attachment for specific travel guidelines):

Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market.

Air Travel: BR/round trip cap

Rental Cars: BR/day all in cap (including taxes and fees) regardless of car class.

Mileage: IRS Standard Rate

Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

Requirement description : **MUST NOTATE ALL DURING NAME CLEAR TO AVOID BEING REDIRECTED**
  • Board Certification OB/GYN Required or Truly BE (05 years of residency completion)
  • Active UT License Highly Preferred
  • Active BLS or PALS Controlled substance Required for credentialing
  • Active DEA Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
  • Must be available for 1 week per month ongoing Required
  • Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date Required
  • Clean Malpractice/background Highly Preferred Must include self inquiry NPDB report at time of name clear.
