Job Description
ReliableVA Solutions LLC is a Talent Search firm and our client is seeking a dedicated Physical Therapist in the Chicago and nbsp;area. and nbsp;
As a Physical Therapist in our clients Home Health team you will be responsible for providing Physical Therapy services to patients in their homes assessing patient functioning levels and developing treatment programs to improve independence and physical/cognitive functioning. and nbsp;
You will also be involved in patient and caregiver education supervision and reporting on patient progress.
Minimum Requirements:
Reliable transportation and auto insurance and nbsp;
Must have IL PT license
1 year of PT exp. (no new grads) and nbsp;
1 year of Home Health or Inpatient Rehab
and nbsp;exp. and nbsp;
OASIS and amp; HCHB highly preferred but will be taught in orientation.
Key Responsibilities
Perform Physical Therapy assessments and develop treatment programs
Record and report patients reaction to treatment and condition changes
Instruct patients and caregivers in therapy programs and device use
Supervise physical therapy assistant and when appropriate the certified nursing assistant
Prepare reports and participate in staff and Agency development activities
Schedule and Shift Details
MondayFriday; 8:00a/9:00a5:00p 1 weekend per month (Saturday and amp; Sunday; day shift w/scheduled patients) NO OnCall
2 holidays (1 summer / 1 winter) and nbsp;
This is a PayPerVisit role and nbsp;
Coverage Area: Chicago (City) and Surrounding Areas
and nbsp;
Travel
Local travel to residential homes Coverage Area: Chicago (City) and Surrounding Areas
