Job Description

Position: Licensed Practical/Vocational Nurse(LPN/LVN) Long Term Care

Start Date: 11/4/2024

Duration: 13 weeks

Location: Iron River MI 49935

Shift Type/Shift Times/Hours/Day: Days 4

Shift Notes: 4 hr days 20 hrs GWW

Weekend Requirements: TBD

OnCall Requirements: None

  • Observe each resident continuously alert to changes in mental and physical wellbeing; notify the Supervisor of pertinent observations and discuss the need to notify the physician; receive Resident Status reports from offgoing shift and give oncoming shift a detailed report of each residents status and events of the shift.
  • Record narcotics on a 30 days basis
  • Accurately receive transcribe and carry out physicians orders.
  • Administer medications and treatments as ordered by the physician and make proper documentation via a computerized system.
  • Perform wound care and know wound vacs.
  • Perform regular duties on the unit such as checking appointments for the day; checking Physical Occupational and Speech Therapy appointments; checking activity board; reminding staff of ambulation lists weights and blood pressures and documenting nutritional charts. Monitor LNA documentation and update LNA clipboard; oversight of infection control practices on the unit; ensure that the unit is clean and hazardfree. Following the chain of command report any problems to the Nurse Manager/Nursing Supervisor. Encourage LNA observations and include them when documenting and reporting.
  • Admit discharge and transfer residents completing documentation and notifications as necessary.
  • Complete 24hour report for his/her assigned unit(s); check new orders; complete list of residents on antibiotics precautions etc.; maintain consistent communication with the Nurse Manager/ Nursing Supervisor regarding resident issues or concerns with staff; differentiating between acute and nonemergency problems; and attend all mandatory staff/educational meetings and nonmandatory meetings as assigned.
  • Communicate with residents families to resolve resident concerns and problems notify family members of new orders and obtain consent when necessary.
  • Receive incoming calls and determine the appropriate response required.
  • Follow State and Federal Regulations regarding the storage usage and documentation of controlled medications including counting at the change of shift; notify the appropriate Supervisor if discrepancies are present.
Submission Requirements
  • 6 months experience in LTC REQUIRED
  • BLS REQUIRED
Profile Requirements:
  • Work History
  • Skills Checklist (pertinent to position) current to 1 year
  • 1 Supervisory Signed Reference (from within the past two years of employment)
  • License/Certifications Must be uploaded with profile (Current NH license and BLS 3years current experience Facility will not pend license/certs)
