Job Description

Adelphi is seeking a Qualified Mental Health Professional in Lane County Oregon to Provide assessment therapy and case management services to adults and children. Collaborate with interdisciplinary teams to develop treatment plans and provide integrated behavioral health services.

Requirements:

Must have valid Oregon state license.

Master s degree in social work psychology or a related field

Must hold a valid QMHP license.

Relevant clinical experience in behavioral health

Know how to navigate EPIC/OCHIN Electronic Health Record system.

Familiarity with community health practices and integrated care models preferred.

Responsibilities:

Conduct comprehensive assessments to identify mental health needs.

Develop implement and evaluate individualized treatment plans.

Provide therapy and counseling to individuals families and groups.

Collaborate with multidisciplinary teams to ensure integrated care.

Maintain accurate and uptodate patient records in the electronic health record system.

Facilitate case management and connect clients with community resources.

Provide crisis intervention and support as needed.

Participate in training and supervision of interns or support staff.
