The professional nurse providing care to medical/surgical patients utilizing all components of the nursing process (assesses develops plans implements and evaluates) in the plan of care.Services Include:
- Skilled observation & assessment
- Wound care
- Injections & IV therapy
- Nursing Case Management
- Netsmart formally Allscripts is the EMR.
Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!! NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better).
NonBillable Orientation: 8hrs
Holiday OT Multipliers: 1.25%
Requirement description :
- Minimum of 2 years CURRENT Home Health experience in Case Management and Hospice Experience
- RN will be working in the Whitefield/Berlin NH area
- RNs will be providing Home Health & Hospice Case Management services to patients of all ages (infant to elderly)
- Netsmart/Allscripts EMR experience strongly preferred
- Mileage paid at current approved Federal IRS Rate (Mileage paid from office each morning)

- Work History
- Skills Checklist (pertinent to position) current to 1 year
- 1 Supervisory Signed Reference (from within the past two years of employment)
- License/Certifications Must be uploaded with profile
CERTIFICATION REQUIREMENTS :
- BLS (AHA)
STATE LICENSE REQUIREMENTS :
- New Hampshire
ADDITIONAL LICENSE REQUIREMENTS : NH or Compact RN license must be inhand at time of upload. Facility will not pend license
Weekend Requirements : No weekends/ No Holidays
