Home Health Physical Therapist (PT)

Equip Human Solutions is a partnering up a growing Home Health organization in Palm Springs CA! Were seeking experienced PTs to provide personalized care to patients in the Murrieta/Temecula territory (Lake Elsinore Wildomar Corona).

Responsibilities:
- Develop and implement personalized physical therapy plans.
- Assess patient progress and adjust treatment plans accordingly.

Requirements
- Bachelors or Masters degree in Physical Therapy
- California Physical Therapy license
- Minimum 1 year of experience in physical therapy

- Current CPR certification
- Valid drivers license and reliable vehicle
