Pay: $35.24 to $48.00 /hourly Relocation assistance

Home Health Physical Therapist Opportunity:

• MondayFriday
  
• Day shift/ varied hours
  
• No weekends no holidays.
  
• Setting: Home Health
  
• Will be needing to travel to see patients.
  
• Paid Mileage reimbursement program
  
• Home Health Physical Therapist will see about 26 patients a day
  

Home Health Physical Therapist Job Description:

• Provide physical therapy treatment and patient care according to hospital state and federal regulations and professional guidelines.
  
• Home Health Physical Therapist develops a Plan of Care including treatments procedures modalities problems and goals.
  
• A visit to the patients residence at least once a month either when the assistant is present to observe and assist or when the assistant is absent to assess relationships and determine that the planofcare is being followed and the goals are being met. Documentation of supervisory visits will be maintained in the patients clinical record.
  

Home Health Physical Therapist Qualifications:

• Current PT License in good standing
  
• BLS certification
  
• 1 year of home health experience required.
