Job Description


Summary: St. Elizabeth is looking for OB coverage for open dates in November. Ongoing dates will be TBD as the facility completes their schedules.


BID DUE DATE: First to Fill!


Rate Caps for Submission:

Malpractice Must be included in rates.

Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly Per hour not OT

Holiday Must be 1.5x the regular rate. Per holiday hour no OT

On Call (Daily) BR3500 Per 24 hours to include 4 hours gratis

24 Hour Holiday Call Rate BRBR Per 24 hours to include 4 hours gratis

Call Back BR01450 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Holiday Call Back BRBR Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

  • 24 hour call (7a7a)
  • Facility Location: 1000 Lincoln St. Fort Morgan CO 80701
  • Setting: Call No Trauma/Not Rated
  • FTE: 1
  • Required Procedures/Job Duties: General OB/GYN
    EMR: Epic
    Support Staff: RN
  • Reason for Coverage: Current locum unable to fill these dates.

EMR Required Training:

  • Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
  • EMR Virtual training Only able to attend if Modules are completed before training. Virtual EMR training must be scheduled prior to the providers first day to avoid the provider missing their first day.

Travel Preferences (see attachment for specific travel guidelines):

Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market.

Air Travel: BR50/round trip cap

Rental Cars: BR/day all in cap (including taxes and fees) regardless of car class.

Mileage: IRS Standard Rate

Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

  • How many patients per day will there be I see there are types of patients seen but needing clarification on the amount of patients per day This is for hospital call only so it is hard to determine how many patients they may see. All depends on how many moms are due to deliver.

  • Board Certified OBGYN Required
  • Active CO license Required or IMLC with LOQ in hand at time of submission
  • Active BLS Required for credentialing
  • Active DEA Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
  • Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date Required
  • Candidate must be available for both November shifts in job description to be considered Required
  • Clean MP/Background Highly Preferred
