Job Description

Position Name: Registered Nurse Hospice Case Manager (Quick Start)

Location: Bedford NH 03110

Start Date: ASAP

Employment Type: Fulltime Travel Contract Temporary

Contract Length:13 weeksAccepting Local and Travel Candidates

Pay Rate: $2200 to $2280 Weekly

Unit: Home Health

Shift: 8 Hours; Days 8a 5p

Schedule: 40 hours / week guaranteed 5 shifts per week MonFri No weekends.

Position Requirements:

  • Registered Nurse: 2 years Required
  • Recent/Current Home Health Nursing experience: 1 year Required
  • Home Care Home Base (HCHB) charting experience Required
  • Case Management experience Highly preferred
  • OASIS experience: 1 year Highly preferred
  • Must have valid Drivers License and Personal Auto Insurance
  • Must be Covid Vaccinated or provide medical or religious exemption

Education License and Certification Requirements:

  • ASN or an equivalent Required
  • BSN or an equivalent Highly preferred
  • RN License for State applying or Compact if accepted Required
  • BLS Certification from AHA or American Red Cross Required

Job Description:

  • Provide and coordinate care for assigned hospice patients in comfort of their homes.
  • Develop and implement nursing care plans including Start of Care and admissions as assigned.
  • Case management and provide clinical oversight to interdisciplinary
  • Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations policies procedures and protocols as well as facility and corporate policies and procedures.
  • Ensure physicians orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status
  • Document and complete charting for all patient visits within 24 hours.
  • Provide Bereavement Support to patient families.
  • Territories to cover: Bedford Nashua Manchester areas
