Job Description
We are hiring an Intake Coordinator for one of our clients in Philadelphia PA.
Job Description:
- Provide assessment intake and screening services in a MAT milieu for prospective patients.
- Point person for all new individuals or facilities seeking MAT services.
- Deliver recovery based clinical services in individual family and group venues for new intakes during the orientation phase of treatment.
- Maintain an active caseload of patients transitioning to the intensive outpatient level of care.
- Complete all required corporate and CBH reports oversee and approve transfer packets oversee and assist intake staff with completing all Biopsy Cho socials.
- Maintain patient census and clinical charts in accordance with NET Steps Pennsylvania Department of Health standards and other city county federal and JCAHO/CARF standards and pursuant to payer contracts and requirements.
