Job Description

Job Title: Registered Nurse LTC (Nursing)
Location: Watertown NY 13601
Shift: Night
Duration: 13 Weeks Contract
Pay rate: $67/hourly
Guaranteed Hours : 36

Job Summary: The Registered Nurse (RN) in a LongTerm Care (LTC) facility plays a crucial role in providing highquality nursing care to residents within the facility. This includes essing planning implementing and evaluating care plans to ensure the physical emotional and social wellbeing of residents.

  • 7 units includes
  • (2 secure dementia units)
  • Post acute rehab a lot of pts from hospital (23 5 7 mix short term rehab )
  • 1 person on 10L Continuous bipap no vents no new trachs tube feeds foleys IVs PICCs ports fresh post op hips/knees/ elective surgeries wound care neb treatments
  • No admissions past 3 pm
  • RN supervisor (they are alone on off shift (nights weekend holidays) so they are tending to any acute change like a fall need to go ER Head to toe essment calling provider getting them over to ED and LPN
  • Dementia Chronic Illnesses Diabetes Rehab

Responsibilities:

  1. essment and Care Planning:

    • Conduct comprehensive essments of residents health status including physical mental and emotional needs.
    • Develop individualized care plans in collaboration with interdisciplinary team members incorporating input from residents families and physicians.
    • Regularly reess residents needs and update care plans accordingly.
  2. Direct Nursing Care:

    • Administer medications and treatments as prescribed ensuring accuracy and adherence to elished protocols.
    • Monitor residents vital signs symptoms and response to treatment intervening promptly to address any concerns.
    • Provide wound care catheter care and other specialized nursing procedures as needed.
    • ist with activities of daily living (ADLs) including bathing dressing toileting and mobility istance.
  3. Doentation and Communication:

    • Maintain accurate and uptodate medical records doenting essments care plans interventions and outcomes.
    • Communicate effectively with residents families and interdisciplinary team members providing education support and guidance as needed.

Qualifications:

  • Graduation from an accredited nursing program with a current license to practice as a Registered Nurse (RN) in the state.
  • Prior experience in longterm care or geriatric nursing preferred.
  • Strong essment critical thinking and communication ss.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Compion empathy and a commitment to providing personcentered care.
  • Willingness to work flexible hours including evenings weekends and holidays as needed.

Physical Requirements:

  • Ability to lift push and pull heavy objects and ist residents with mobility.
  • Stamina to stand for long periods and perform physical tasks ociated with resident care.
  • Manual dexterity and visual acuity to administer medications and perform nursing procedures accurately.

Working Conditions:

  • Work environment primarily indoors within a longterm care facility.
  • Exposure to potentially infectious agents and hazardous materials requiring adherence to infection control protocols and safety precautions.
  • Interaction with residents who may exhibit challenging behaviors or have complex medical needs necessitating patience understanding and effective communication ss.
