Job Description
Client will consider a relocation bonus of xx2000 if a candidate moves to Lawton OK in order to accept the position. Candidate must have a new Lawton address and must relocate in order to be eligible. (Not Elgin Cache or the surrounding areas).
Salary Range: xx24.50 xx27.56/hour (based on experience)
The Physical Therapy istants (Home Health) primary function is to perform selected physical therapy procedures and
related tasks under the direction and supervision of a registered Physical Therapist. The istant will
treat patients with modalities and exercise in accordance with patients medical needs and with an
approved treatment plan and will understand their limitations as an istant. The istant will ist in
maintaining the department in a neat and orderly manner. The istant will complete all doentation
of daily notes and discharge data. Patients cared for may include infants pediatrics adolescents adult
and geriatric.
Comanches NonCompete Policies:
- AHSA is not to knowingly consider a former direct employee of CCMH who was employed within the last 6 months. They are not eligible for hire through AHSA.

- Must have permanent Oklahoma State Licensure as a Physical Therapy istant or have Oklahoma Temporary Permit under direct supervision pending permanent licensure. Required
