As the Implementation Coordinator for the NE TN GROWSS project your primary responsibility is to serve as the key point of contact for identified practices and support the implementation of telebehavioral health services. You will collaborate with providers to develop and optimize workflows support data management and ensure continuous quality improvement (CQI) processes are met.
Key Responsibilities:- Serve as primary point of contact for the Identified Practices.
- Coordinate with IP points of contact to support approach to integrated telebehavioral health.
- Provide support in developing implementation plans which includes but is not limited to coaching workflow shifts to increase fidelity and relationship building with providers.
- Support data collection acquisition and effective extraction.
- Collaborate to elish quarterly CQI and to support IPs in addressing quarterly KPIs for improvement on CQI metrics.
- Connect to additional T&TA as needed such as with ZERO TO THREE and/or HMG!
- Bachelors degree in Healthcare Administration Public Health or a related discipline (required); Masters preferred.
- At least 3 years of experience in healthcare settings.
- Proficiency in Microsoft Office Google Drive and familiarity with data evaluation programs.
- Experience working in a medical or clinical setting is preferred.
- Excellent communication ss both oral and written.
- Strong organizational ss and the ability to work independently.
- Experience working in interdisciplinary teams and effectively managing projects.
