Job Description
Overview:
Key Responsibilities:
- Conduct comprehensive assessments of clients needs and develop personalized care plans
- Provide individual and group counseling to address emotional behavioral and psychological issues
- Offer crisis intervention services during emergencies and critical situations
- Collaborate with healthcare professionals and community resources to coordinate holistic care for clients
- Advocate for clients rights and access to essential services and benefits
- Educate clients and their families about available support programs and resources
- Participate in multidisciplinary teams to develop and implement intervention strategies
- Conduct home visits and outreach activities to connect with marginalized populations
- Document client interactions progress notes and treatment plans in compliance with regulatory standards
- Evaluate the effectiveness of interventions and modify strategies as needed
- Engage in continuous professional development and stay updated on social work best practices
- Contribute to community outreach initiatives and awareness campaigns
- Network and build partnerships with other social service organizations and community stakeholders
- Adhere to ethical and legal guidelines in all interactions and decisionmaking processes
- Support and mentor junior social work staff and interns
Required Qualifications:
- Bachelors or Masters degree in Social Work or related field
- Licensure as a professional social worker (LCSW LMSW etc.) depending on state requirements
- Demonstrated experience in conducting client assessments and developing care plans
- Proficiency in providing individual and group counseling using evidencebased practices
- Strong understanding of crisis intervention techniques and traumainformed care
- Ability to advocate for clients needs and navigate social service systems
- Excellent communication and interpersonal skills for effective collaboration
- Sound knowledge of community resources and support programs
- Proficient in using electronic health records and case management software
- Compliance with professional and ethical standards set by social work associations
- Ability to work with diverse populations and address cultural competency challenges
- Capacity to handle highpressure situations with empathy and professionalism
- Commitment to continuous learning and staying updated on social work trends
- Experience in mentoring or supervising social work staff or interns
- Understanding of legal and confidentiality requirements in social work practice
