Job Description

RN Case Manager

Overview:

The role of RN Case Manager is crucial in facilitating comprehensive healthcare coordination for patients ensuring efficient and effective use of resources while promoting optimal patient outcomes. This role involves collaborating with multidisciplinary teams to assess plan implement coordinate monitor and evaluate healthcare options and services for patients. RN Case Managers play a vital role in advocating for patients managing care plans and ensuring compliance with healthcare regulations.

Key Responsibilities:

  • Conduct comprehensive clinical assessments of patients healthcare needs and develop individualized care plans.
  • Coordinate and facilitate healthcare services across various providers to ensure seamless transitions of care.
  • Advocate for patients to ensure access to necessary resources and services while promoting patient autonomy.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as necessary.
  • Collaborate with healthcare professionals to ensure compliance with healthcare regulations and standards.
  • Provide patient and family education on healthcare management and resources.
  • Participate in interdisciplinary team meetings to discuss patients progress and care plans.
  • Utilize electronic health records and documentation systems to maintain accurate patient records.
  • Assist in discharge planning and postacute care coordination.
  • Conduct regular followup assessments and communicate with patients to assess their ongoing needs.

Required Qualifications:

  • Active Registered Nurse (RN) license in the state of practice.
  • Bachelor of Science in Nursing (BSN) or higher preferred.
  • Certification in Case Management (CCM) or willingness to obtain within a specified timeframe.
  • Minimum of 2 years of clinical nursing experience in a healthcare setting.
  • Demonstrated knowledge of healthcare regulations reimbursement and discharge planning.
  • Strong critical thinking and problemsolving skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team in a fastpaced environment.
  • Proficiency in using electronic health records and healthcare software.
  • Commitment to promoting patientcentered care and advocacy.
