Overview:

Key Responsibilities:

- Monitor and evaluate the effectiveness of care plans and make adjustments as necessary.

Required Qualifications:
- Active Registered Nurse (RN) license in the state of practice.
- Bachelor of Science in Nursing (BSN) or higher preferred.
- Certification in Case Management (CCM) or willingness to obtain within a specified timeframe.
- Minimum of 2 years of clinical nursing experience in a healthcare setting.
- Demonstrated knowledge of healthcare regulations reimbursement and discharge planning.
- Strong critical thinking and problemsolving skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team in a fastpaced environment.
