Job Title: Registered Nurse Case Manager
Location: Oakland CA 94602
Company: United Healthcare System
Job Type: 13 Weeks
Shift: Day Shift 8:00am 4:30pm
Salary: $71:00 per hour
Responsibilities:
- Conduct comprehensive assessments to determine the healthcare needs of patients.
- Develop and implement individualized case management plans to ensure optimal patient care.
- Collaborate with healthcare professionals social workers and other stakeholders to coordinate and facilitate patient care.
- Monitor and evaluate patient progress and adjust care plans as needed.
- Advocate for patients to ensure access to appropriate resources and services.
- Maintain accurate and detailed documentation of case management activities.
Requirements:
- Registered Nurse (RN) licensure.
- Experience in case management or care coordination.
- Knowledge of healthcare resources insurance and reimbursement processes.
- Basic Life Support (BLS) certification.
