Job Description
Position Name: Registered Nurse Hospice Case Manager
Location: Lorain OH 44053
Start Date: ASAP
Employment Type: Fulltime Travel Contract Temporary
Contract Length:13 weeksAccepting Local and Travel Candidates
Pay Rate: $2120 to $2160 Weekly
Unit: Home Health Hospice
Shift: 8 Hours; Days 8a 5p or Evenings 4 pm to 12.30 am
Schedule: 40 hours / week guaranteed 5 shifts per week MonFri
Position Requirements:
- Registered Nurse: 2 years Required
- Recent/Current Hospice Nursing experience in a home health setting: 1 year within last 3 years Required
- Home Care Home Base (HCHB) charting experience Required
- OASIS experience: 1 year Highly preferred
- Must have valid Drivers License and Personal Auto Insurance
- Must be Covid Vaccinated or provide medical or religious exemption
Education License and Certification Requirements:
- ASN or an equivalent Required
- BSN or an equivalent Highly preferred
- RN License for State of KY or Compact Required
- RN License for State of IL Highly preferred
- BLS Certification from AHA or American Red Cross Required
Job Description:
- Provide and coordinate care for assigned hospice patients in comfort of their homes.
- Develop and implement nursing care plans including Start of Care and admissions as assigned.
- Support patient recovery and ability to stay at home enabling them to spend quality time with their loved ones and doing the activities they enjoy.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations policies procedures and protocols as well as facility and corporate policies and procedures.
- Ensure physicians orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting for all patient visits within 24 hours.
- Provide bereavement support to patients families as needed.
- Territories to cover: Lorain OH and surrounding cities
