Shift Type/Shift Times/Hours/Day: Nights7:00 PM 7:30 AM 12

Shift Notes: 7:00 PM 7:30 AM/36 hours scheduled/24 hours guaranteed/the client is allowed to cancel 1 shift per week/Must be willing to float to units where experienced

Weekend Requirements: Weekend schedule to be determined by facility

OnCall Requirements: Oncall schedule to be determined by facility

COVID19/FLU Requirement: Flu vaccination REQUIRED EFFECTIVE 9/16 **Please add vaccination status in the submission notes**

Traveler Policy: No candidates with an AZ home address on their drivers license will be considered please include a copy in the submission

• Learning modules must be completed before the start of the assignment.
• Nonbillable hours have been adjusted to reflect 16 hours for each assignment.
• Client or Vendor may cancel an Assignment in progress with 7 days written notice without penalty.

** The client will not approve RTO within the first two weeks or holidays. Can not exceed 7 days.

The client has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in. This helps us get candidates presented quicker as we all know these orders open/close quickly!

This form needs to be 100% accurate and have all requests noted in the highlights section.

• Start Date should be on or very quickly after the Order Start Date listed.
• All RTO should be listed on the cover page and must be less than 7 days.
• RTO will not be approved in the first 2 weeks of the assignment.
• All facilities the traveler is open to working in should be listed on the submission.
• If interested in any location please include what states (I.E. Any AZ Any CO Any state etc.)
• All shifts the traveler is open to working on should be listed on the submission.
• All departments the traveler is open to working must be checked on the cover page.
• All candidates submitted should be preclosed and ready to accept.
• Please let us know if the traveler withdraws or is no longer available.

• Minimum 24 months of current Level I Trauma ICU experience REQUIRED
• MICU experience required. Must have previous continuous ICU experience for the last 4 years REQUIRED
• May float to the other ICU units as needed NSICU CVICU and STICU REQUIRED
• Certifications must be American Heart Association OR American Red Cross and active upon submission MUST BE INCLUDED IN SUBMISSION REQUIRED
• No Nursys Hits REQUIRED
• Travel experience Preferred

• Skills Checklist specific to the discipline (must be less than a year old)
• Active License (verification must be less than 30 days old)
• Certifications (must be active at the time of submission)
