Overview:
As a Physical Therapist on our client s Home Health team you will provide inhome Physical Therapy services to patients assess their physical and cognitive functioning and develop treatment programs aimed at improving independence. Your role will include patient and caregiver education progress reporting and supervision of other healthcare assistants.
Key Responsibilities:
- Perform Physical Therapy assessments and create treatment programs.
- Record and report patient reactions to treatments and any changes in their condition.
- Instruct patients and caregivers on therapy programs and the use of devices.
- Supervise the physical therapy assistant and certified nursing assistants as appropriate.
- Prepare reports and participate in staff and agency development activities.
- Healthcare Setting (1 years)
- Masters degree or higher
- Valid Physical Therapist certification/license in the state of Illinois (IL)
- Valid Regular Driver License (DL) certification/license
Schedule & Shift Details:
- Monday to Friday 8:00 AM/9:00 AM 5:00 PM.
- 1 weekend per month (Saturday & Sunday day shift with scheduled patients).
- 2 holidays (1 summer 1 winter) no oncall requirements.
Travel:
- Local travel to patients residential homes is required.
