NBO: all hours billed
Multiplier: 1.25x
OT: billed at straight hrly
Requirement description :
Submission Requirements
1. work history going back 35 years
2. skills checklist within the year
3. supervisory references if applicable (required during onboarding)
4. MI license & certificate(s)
5. covid vaccination card
CERTIFICATION REQUIREMENTS :
STATE LICENSE REQUIREMENTS :
ADDITIONAL LICENSE REQUIREMENTS :
Weekend Requirements : rotation usually 1 weekend off/month
On Call Requirements : None

- 2 years experience in SNF/LTC as a LPN
- Please note there is no public transportation. All travelers must have their own vehicle or a rental
- travel experience is desired
- Agency MUST have an active MI iCHAT account
- Agency MUST be authorize to schedule Fingerprinting
