Job Description
Job Title: RN Women and Childrens services
Location: Crownpoint NM 87313
Start Date: Nov 2024
Duration: 3 6 months
Shift Schedule: 8 hours shift
Qualification:
License: Any state RN license accepted
Certifications: BLS ACLS PALS recommended
Experience: RN shall have thirtysix 36) months of nursing experience with a minimum of two (2) years in women and children
Responsibilities:
- Provide adult and pediatric case management in Ambulatory Care and Women and Childrens services.
- Apply nursing processes for individualized goaloriented care.
- Develop implement and evaluate patient care coordination and case management programs.
- Collaborate with healthcare providers and services to ensure timely and costeffective patient care.
- Manage clinic and followup visits coordinating with multidisciplinary teams and conducting chart reviews.
- Facilitate patient care conferences and provide interpretation as needed.
- Monitor and evaluate the effectiveness of care coordination through chart reviews.
- Assist providers with treatment plans and coordinate appointments for optimal clinical outcomes.
