Job Description
Job Title: RN Ambulatory
Location: Crownpoint NM 87313
Start Date: Nov 2024
Duration: 3 6 months
Shift Schedule: 8 hours shift
Qualification:
License: Any state RN license accepted
Certifications: BLS ACLS (PALS not mandatory)
Experience: r shall have thirtysix 36) months of nursing experience with a minimum of two (2) years in Ambulatory
Responsibilities:
- Provide comprehensive adult and pediatric case management in Ambulatory Care and Women and Childrens services.
- Apply nursing processes to deliver individualized goaloriented care.
- Create implement and assess patient care coordination and case management programs.
- Collaborate with primary care providers nurses public health social services and multidisciplinary teams for timely and costeffective patient care.
- Coordinate all clinic and followup visits including chart reviews.
- Organize and facilitate patient care conferences and provide interpretation as needed.
- Review patient charts to assess the effectiveness of care coordination and documentation.
