Position Title: Staff Pharmacist

Position Overview:
The Staff Pharmacist under the general supervision of the Director of Pharmacy plays a critical role in ensuring the safe and effective delivery of pharmaceutical services within our institution. This position involves performing a wide range of professional functions in accordance with the Vermont Board of Pharmacy Rules and Regulations. The Staff Pharmacist will be responsible for dispensing medications providing drug information overseeing medication management and ensuring compliance with all relevant regulations and standards. The role also involves collaborating with healthcare professionals to optimize patient care and outcomes.
Key Responsibilities:
- Medication Dispensing & Verification: Accurately dispense prescribed medications ensuring correct dosages forms and instructions are provided to patients. Verify prescriptions for potential drug interactions contraindications and allergies.
- Patient Consultation: Provide expert advice and consultation to patients and healthcare providers regarding medication therapy including proper administration potential side effects and interactions.
- Medication Therapy Management: Conduct medication reviews and therapy management to optimize patient outcomes adjusting prescriptions in collaboration with physicians as needed.
- Regulatory Compliance: Ensure compliance with all state and federal regulations including Vermont Board of Pharmacy Rules and Regulations maintaining accurate records and documentation.
- Inventory Management: Manage and maintain the pharmacys inventory ensuring the availability of medications proper storage and adherence to safety standards.
- Collaborative Care: Work closely with physicians nurses and other healthcare professionals to develop and implement patientspecific pharmaceutical care plans.
- Continuing Education & Training: Stay current with developments in the field of pharmacy through continuing education and training and assist in the education and training of pharmacy staff and students.
- Quality Assurance: Participate in quality assurance initiatives to improve pharmacy services including medication error prevention and adherence to best practices.
- Technology Utilization: Utilize pharmacy management software and other relevant technology to ensure accurate data entry medication tracking and reporting.
Requirements
Education & Qualifications:
- Degree: Bachelor s Degree in Pharmacy from an accredited School of Pharmacy. Advanced degrees or certifications in pharmacy practice or related fields are highly desirable.
- Licensure: Active Vermont Pharmacy License in good standing.
- Skills: Proficient in computer use and data entry with strong oral and written communication skills. Ability to effectively interact with patients healthcare providers and team members.
Experience:
- Minimum Requirement: At least one year of professional experience in a pharmacy setting preferably within a hospital or other healthcare institution.
- Preferred: Experience in an institutional or clinical pharmacy setting with a strong understanding of regulatory compliance patient care and interdisciplinary collaboration.
