We are looking for a Registered Nurse to provide care for people who are terminally ill. The ideal candidate will have a passion for providing quality care to patients in a home health setting and be able to demonstrate excellent communication and organizational skills. The successful candidate will be responsible for providing assessments developing care plans administering medications and monitoring patient progress.
Responsibilities:
- Frequently visiting patients to monitor vital signs and physical comfort levels.
- Make sure the clients medications are refilled.
- Informing physicians about changes in patients conditions.
- Notifying attending physicians hospice staff members and family members of patients deaths when they occur.
- Collaborating on terminally ill patients care with physicians social workers chaplains nursing assistants and other stakeholders.
- Preparing and adjusting patient care plans as needed.
Weekly schedule:
- Monday to Friday
- 8:00 am to 4:30 pm
- 40 hours
- 13 Weeks
Requirements
Requirements:
- Has knowledge and skills in dealing with death and dying.
- A minimum of 12 years experience as a home care hospice nurse.
- Valid RN license in MA
- BLS
- US Drivers license
