Position Summary:

Provide Case Management leadership and management for hospitals within a market of Client. The position will be responsible for the management or delegation of daytoday operations including directing and planning. Formulate and implement operational strategies and initiatives in the case management program. Ensure staff competencies in all areas of care management including utilization review transition/discharge planning and social work services. Provide specialized expertise ensure resource alignment with systemwide initiatives and update appropriate parties on case management strategies initiatives outcomes and issues. Successfully partner with physicians and others to ensure delivery of evidencebased care and reduction of avoidable clinical variation in care.

Key Responsibilities:

• Manage and delegate daytoday operations of the case management program.
• Formulate and implement operational strategies and initiatives.
• Ensure staff competencies in utilization review transition/discharge planning and social work services.
• Provide specialized expertise and ensure resource alignment with systemwide initiatives.
• Update appropriate parties on case management strategies initiatives outcomes and issues.
• Partner with physicians and other stakeholders to ensure delivery of evidencebased care and reduce clinical variation.
• Collaborate with Hospital Executive Leadership Nursing Medical staff and other department stakeholders to facilitate care in a safe environment.

Reports to: Chief Financial Officer

Minimum Education:

• Bachelors degree in Nursing Required
• Masters degree in Nursing Healthcare Administration or Business Administration Preferred

Required Skills:

• Working knowledge of Care Management department functions including Joint Commission and Conditions of Participation (COP) requirements.
• Good understanding of Medicare Medicaid and other thirdparty payment systems.
• Ability to critically think research solutions and challenge the status quo.
• Advanced computer skills including Microsoft Office; especially Word Excel and PowerPoint.
• Strong collaborative work style and commitment to excellence in job performance.
• Ability to work professionally with confidential proprietary data and information while maintaining confidentiality.
• Excellent interpersonal skills and the ability to interact effectively and professionally with individuals at all levels both internal and external.
• Excellent organizational skills with attention to detail and the ability to plan implement and manage multiple projects simultaneously within inflexible time frames.
• Must be selfmotivated and have the ability to work within the established policies procedures and practices prescribed by the organization.
• Proven leadership with the ability to navigate conflict develop teams and delegate.
• English sufficient to provide and receive instructions/directions.

Required Certifications:

• BLS certification from the American Heart Association (AHA) or American Red Cross (ARC) Required within 30 days of hire if not current upon hire.
• Licenses: State of Tennessee or MultiState Registered Nurse licensure Required.
• Certification in Case Management such as CCM Preferred.

Minimum Work Experience:

• Minimum four years management experience directing a Case Management Department in an acute care hospital.
