Job Description

Position Overview:

We are seeking a highly organized and empathetic Intake Specialist to join our team. The Intake Specialist will be the first point of contact for new clients responsible for gathering essential information assessing client needs and ensuring a seamless onboarding process. The ideal candidate will have excellent communication skills a keen eye for detail and the ability to handle sensitive information with confidentiality.

Key Responsibilities:

Serve as the initial point of contact for new clients providing a welcoming and professional experience.
Collect and accurately record client information including personal details medical history and other relevant data.
Conduct initial assessments to determine client needs and eligibility for services.
Answer and manage incoming calls emails and inquiries promptly and courteously.
Schedule appointments and coordinate with other team members to ensure timely service delivery.
Maintain detailed and organized records of client interactions and information.
Provide clients with information about available services processes and next steps.
Collaborate with other departments to ensure smooth client transitions and followup.
Handle confidential information in accordance with company policies and legal regulations.
Assist with administrative tasks and special projects as needed.

Qualifications:

High school diploma or equivalent; additional education or certification in healthcare social work or a related field is a plus.
Proven experience in a customer service or intake role preferably in a healthcare or social services setting.
Excellent interpersonal and communication skills both written and verbal.
Strong organizational skills and attention to detail.
Ability to handle sensitive and confidential information with discretion.
Proficient in using computer systems and software including Microsoft Office Suite and customer relationship management (CRM) systems.
Ability to work independently and as part of a team.
Strong problemsolving skills and a compassionate approach to client interactions.
