Licensed Practical Nurse LPN Hospice

Mobile Health Team Inc. a staffing agency has an opening for a Licensed Practical Nurse(LPN). We are looking for someone with a passion for people.

Assignment Start Date: 7/9/2024

13 weeks Days 8hour shift 40 hours per week guaranteed

The Licensed Practical Nurse (under the direction of an RN Case Manager) is responsible for direct nursing care to Hospice patients and for educating patients and families through the care plan established by the interdisciplinary team.

DUTIES

• Making home visits under the direct supervision of the RN Case Manager to assist with the provision of care to meet patient/family needs.
• Communicate changes in patient condition and needed changes to the plan of care to appropriate team members and caregivers.
• Assists the RN and/or medical provider in performing specialized duties related to endoflife care.
• Observe document and report to the RN as appropriate the patients physical and emotional needs and reactions to care.

EXPERIENCE/EDUCATION/QUALIFICATION

Experience:

• A minimum of 2 years of previous LPN experience required

Education:

• A graduate from an approved school of practical nursing is preferred

Licenses Certifications and/or Registration:

• Must be a Licensed Practical Nurse in Vermont and New Hampshire.

EMR System: EPIC experience preferred

Shift: Day shift 8:00 am 4:30 pm Monday Friday; weekend oncall days rotation
