Job Title: Medical Director Hospice
Location: Cleveland OH 44101
Salary: $145.00 h/r $150.00 h/r
Schedule and Shift Details:
- Hours: 810 hours per week
- Type: Contract (1year duration)
- Oncall: Potential oncall responsibilities
- Travel: Occasional visits to patient homes community locations and facility settings for direct client/customer interaction
Responsibilities:
- Provide medical oversight and direction for hospice care services.
- Develop and implement comprehensive care plans tailored to the needs of hospice patients.
- Conduct patient assessments and collaborate with the interdisciplinary team to ensure highquality patient care.
- Monitor patient progress adjusting care plans as necessary to ensure comfort and quality of life.
- Serve as a liaison between hospice staff patients and their families offering guidance and support.
- Participate in interdisciplinary team meetings and provide clinical guidance to the hospice care team.
- Ensure compliance with all relevant federal state and local regulations as well as hospice policies and procedures.
- Educate staff patients and families about hospice care principles and practices.
- Maintain accurate and detailed patient records and documentation.
Requirements:
- Education: Medical degree (MD or DO) from an accredited institution.
- Certification: Board certification in Hospice and Palliative Medicine preferred.
- Drug Enforcement Administration (DEA)
o National Provider Identifier (NPI)
o American Heart Association Basic Life Support (AHA BLS) - Licensure: Valid medical license to practice in the state of Ohio.
- Medical Doctor (MD) in Ohio (OH)
o Doctor of Osteopathic Medicine (DO) in Ohio (OH)
Boardcertified in Hospice and/or Palliative Medicine - Experience: Prior experience in hospice or palliative care preferred.
- Skills:
- Strong clinical and diagnostic skills in palliative and endoflife care.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with an interdisciplinary team.
- Compassionate approach to patient care and family support.
- Strong organizational and time management abilities.
- Physical Requirements: Ability to travel to patient locations and perform physical examinations.

The Medical Director will be reimbursed for hours worked at a rate of $150.00/hour including but not limited to
interdisciplinary meetings peer to peer education continuing education and program development/oversight.
If applicable oncall hours will be reimbursed at the rate of $75.00/day.
