Were currently partnered with a growing Home Health organization looking for RNs in the Santa Cruz County area.

This role is paypervisit which directly allows for higher earning potential and more autonomy. This role offers PPV rates $9095/point (plus mileage) with an admin rate of $65/hr.

Key Responsibilities:

• Maintain 30 points per week.
  
• Conduct comprehensive assessments of patients health status and develop individualized care plans.
  
• Administer medications treatments and therapies as prescribed by the physician.
  
• Monitor patients conditions and report any changes or concerns
  
• Educate patients and their families on disease management medication adherence and selfcare techniques.
  
• Collaborate with other healthcare professionals including therapists social workers and home health aides to ensure holistic care delivery.
  
• Maintain accurate and timely documentation of patient assessments interventions and outcomes in electronic health records.
  

Requirements

• Current Registered Nurse (RN) license in the state of California
  
• Reliable and insured vehicle and valid drivers license.
  
• Minimum of 1 year of clinical nursing experience preferably in home health or a related setting.
  
• Strong clinical assessment and critical thinking skills.
  
• Excellent communication and interpersonal skills.
  
• Ability to work independently and as part of a multidisciplinary team.
  
• Proficiency in electronic health record documentation.
