Registered Nurse Home Care

Overview:

We are looking for Registered Nurses for Home Health Care. Responsible for patient care in home settings conducting assessments coordinating with physicians and delivering skilled nursing care.

Duties:

• Perform home care assessments.
• Develop individualized care plans.
• Implement the Plans of Treatment
• Provide skilled nursing care.
• Communicate with physicians and interdisciplinary team.
• Document patient care accurately.
• Participate in quality improvement initiatives.
• Adhere to infection control protocols.

Requirements:

• 1 year of professional experience as an RN (Home Health experience preferred)
• Active RN License in the State of PA
• Associate degree (or higher) in Nursing
• BLS and CPR Certification (if you dont have this we will help you get it!)
• Valid Drivers License Reliable Mode of Transportation and Auto Insurance
