Shift Notes: 8:00 AM 4:30 PM/40 hours guaranteed

Weekend Requirements: No weekends

OnCall Requirements: No Call

Travel Policy: na

Territory: Within 15 miles (may increase if the census is low and they need to offer the 40 hours)

Mileage: Reimbursed at the standard IRS rate.

The Physical Therapist (PT) evaluates the physical therapy needs of assigned patients establishes goals with the patient and plans develops and implements individual physical therapy programs designed to improve patient mobility and independent function. The PT provides age and developmentallyappropriate care to pediatric to geriatric patients. Effectively interacts with patients doctors and other healthcare members.

• Assess plan treat evaluate and implement a POC for all clients seen
• Demonstrates understanding of admission process treatment costs coding financial support & appropriate billing
• Communicates and interacts with patients families physicians and staff demonstrating behavior consistent with the hospitals core values
• Completes documentation of all patient care activities promptly in the electronic medical record.
• Ability to transfer and position patients.

• 1 year Home Health experience REQUIRED
• Valid drivers license and car insurance REQUIRED
• OASIS and start of care experience strongly preferred
• The manager will review local candidates! If the candidate is too far from the territory they will redirect.
