Job Description
Summary
- Department: and nbsp;MercyOne Waterloo Medical Center(4G)
- Shift timing: and nbsp;07:00 19:00 Hours Monday to friday (3 Days/12 Hours) and nbsp;
- Per Week: 36 Hours.
Roles and amp; Responsibilities
- Implement and monitor patient care plans. Monitor record and communicate patient condition as appropriate.
- Serve as a primary coordinator of all disciplines for wellcoordinated patient care.
- Note and carry out physician and nursing orders.
- Assess and coordinate patients discharge planning needs with members of the healthcare team.
