Job Description
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will play a crucial role in providing support and resources to patients and their families to ensure holistic care.
Responsibilities
Conduct assessments to identify patients psychosocial needs
Develop and implement care plans in collaboration with the healthcare team
Provide counseling and emotional support to patients and families
Assist in discharge planning and coordination of posthospital care
Collaborate with community resources for patient referrals
Maintain accurate and confidential patient records
Skills
Proficiency in medical terminology and data collection
Experience in Pediatrics Assisted Living Nursing Home or Home Community Care settings
Knowledge of coding and billing practices
Familiarity with Applied Behavior Analysis and Behavioral Therapy techniques

Pay: $49.00 $55.00 per hour
